Please read the policy and review the material on the Guide One site following these instructions.  You will then provide the following information to the Church office (complete the forms in the policy document, print them off, and return them to the church office):

1. Completed Form A
2. Completed Form B
3. A copy of your driver's license
4. A copy of your Guide One certificate.
5. Completed Van Orientation
